Our Care Model

This website was designed to help you and your loved ones tackle some of the common but difficult challenges seen in Dementia. A diagnosis can be made by an Adult or Family Medicine doctor (primary physician) after hearing your story, performing a bedside cognitive examination, and ordering simple blood work along with an imaging study like a CT scan.

A referral to the Memory Care clinic may be necessary to help confirm a diagnosis, help with challenging situations, and treatment optimization.

Here is what to expect:
Please note that not every patient will see all providers listed below

  • Adult Family Medicine
    - Performs physical exam and medication
    evaluation
    - Orders labs and imaging studies

    Primary Physician/Geriatrician

Your primary care physician is your health care co-pilot. They deliver your overall care for non-dementia-related conditions, and partner with our Memory clinic. You generally start with an initial evaluation with your primary physician for a brief cognitive assessment, review reversible conditions that may affect cognitive functions, review high-risk medications that affect cognitive functions, & order initial labs and possibly brain imaging.
