The North Valley Memory Care Clinic offers screening, diagnosis, and treatment plans for our members with conditions such as memory loss, mild cognitive impairment, and dementia. Our team of Neurologists and Nurse Practitioners can offer medications to help with the treatment of behaviors associated with dementia, and help patients and families in the office, via phone or secure messaging. Our Medical Social Workers provide education, support, and community resource referrals related to dementia & caregiving as well as Life Care Planning.
